TSLRP Pays $50,000, Then Keeps Paying. But Awards Are Rationed by Site.
Program and regulatory figures verified October 9, 2026. Details change; confirm your scenario with us.
The programme most coverage of Tennessee loan repayment actually means. Smaller than the family medicine incentive, open to far more disciplines, and rationed in a way almost nobody writes about.
The award
From the Department of Health's own announcement: "Maximum TSLRP award is up to $50,000 for an initial 2-year service obligation contingent upon funding availability."
★ The funding caveat is the Department's, not ours. We quote it and make no claim about whether any given cycle is funded.
★ The part that is easy to miss: it renews
"TSLRP recipients in good standing are eligible to reapply for a maximum of up to $20,000 per year in continued loan repayment awards after fulfilling their initial 2-year service obligation and as long as they have outstanding educational loans to repay contingent upon funding availability."
So $50,000 is the entry award, not the ceiling on what a career in a designated site can draw. Against the $200,000 family medicine route.
★★ The sentence that changes the odds
"TSLRP awards are limited to only one provider per site and three per health care system per year."
Additional providers may be considered, and the announcement lists what that turns on: the severity of the local primary care workforce shortage, funding availability, and how many TSLRP recipients already practise at that location. Priority goes to sites that do not currently have a TSLRP recipient.
★★ Read that as a practical instruction. Two physicians at the same clinic are competing with each other, and a large health system can only place three in a year. If you are weighing two offers and loan repayment matters to you, ask the employer whether anyone at that site already holds a TSLRP award. That one question tells you more than the award ceiling does.
★★ Who is eligible, and it is broader than "doctors"
The Department publishes its own list, and physicians are one row of it:
| Discipline | Detail from the announcement |
|---|---|
| Primary care physician | MD or DO in family medicine, general internal medicine, paediatrics, obstetrics and gynaecology, geriatrics. ★ Physicians who have not completed a residency training programme are not eligible |
| ★★ Dentist | DDS or DMD, general practice and paediatrics |
| Advanced practice nurse | primary care certified nurse practitioner or certified nurse midwife |
| ★ Registered nurse | RN at BSN level: added in the revised announcement |
| Physician assistant | primary care PA across adult, family, paediatric, geriatric, women's health, psychiatry, mental health |
| Behavioural and mental health | psychiatrists, licensed clinical social workers, psychiatric nurse specialists, health service psychologists, marriage and family therapists, licensed professional counsellors |
| Pharmacist (Pharm-D) | ★ not those who only dispense or work in retail settings; must work in an integrated care approach |
★★ Dentists being named outright matters, because dental professionals searching for Tennessee help are usually pointed at nothing. The dentist page.
The site, and the hours
| Requirement | Detail |
|---|---|
| Site | ambulatory primary care public, non-profit or private not-for-profit entity in a federally designated HPSA |
| ★ Full-time | 40 hours a week, with no more than 8 hours on practice-related administrative duties |
| Part-time | 20 to 39 hours, choosing a 2 or 4 year obligation |
| Minimum commitment | a service agreement of at least two years |
| Also required | serve Medicaid and Medicare patients; a CoverKids agreement under Title XXI; a sliding fee scale for the uninsured, posted publicly |
| Excluded | an existing unserved service obligation, a breached health professional contract, or any active Primary Care Loan |
| Application | online, powered by REDCap, only while the cycle is open |
★ The 8-hour administrative cap is the detail that catches people. A full-time post that is heavily administrative may not meet the test even at 40 hours.
★★ A corrective: there are two live versions of this announcement
This matters if you are reading other coverage.
The Department has an older TSLRP announcement still sitting on its website, under program-areas/rural-health. It loads normally. It describes the November 2020 to January 2021 application cycle and names a coordinator who no longer holds the role.
The current announcement, revised November 2024, differs in three ways that change who qualifies:
| Fact | Older document | ★ Current document |
|---|---|---|
| Eligible site | HPSA or a Federally Qualified Health Center or Rural Health Clinic | ★★ an HPSA; the FQHC and RHC wording is gone |
| Commitment | minimum 2, maximum 5 years | minimum 2 years; the five-year cap is gone |
| Registered nurses | not listed | ★ RN at BSN level listed |
★★ So if you read somewhere that an FQHC placement qualifies you for TSLRP, that is the older document talking. The current one conditions the site on HPSA designation. We corrected this page after checking both, and we are leaving the correction visible because the stale version is still the one search engines surface.
★ Verify the site with the Department before you plan around an award. It is their determination, not ours.
★ The cycle, dated honestly
The current announcement gives its application cycle as 25 November 2024 to 31 January 2025. That is the most recent window we can show you from a primary source.
Secondary coverage points at a Fall 2026 cycle. We did not confirm it on a Department page, so we are not printing those dates as fact. Ask the State Office of Rural Health and Health Access for the current window.
What it means for a mortgage
A $50,000 initial award plus renewals changes a medium-term debt trajectory. It does not reduce the balance on the day you are underwritten, and it is conditional on funding and on completing the obligation.
Buy on the salary; treat the award as upside. How debt is counted.

Frequently asked questions
How much does the Tennessee State Loan Repayment Program pay?
Up to $50,000 for an initial two-year service obligation, contingent upon funding availability. Recipients in good standing may then reapply for up to $20,000 per year in continued awards after fulfilling that initial obligation, for as long as they have outstanding educational loans to repay. How many TSLRP awards are available at one practice site?
The Department's announcement states that awards are limited to only one provider per site and three per health care system per year. Additional providers may be considered depending on the severity of the local workforce shortage, funding availability, and how many recipients already practise at that location, with priority given to sites that do not currently have a recipient. Are dentists eligible for TSLRP?
Yes. The Department's eligible-practitioner list names dentists holding a DDS or DMD in general practice or paediatrics, alongside primary care physicians, advanced practice nurses, registered nurses at BSN level, physician assistants, behavioural and mental health professionals, and pharmacists working outside retail dispensing. Can a resident apply for TSLRP?
No. The announcement states that physicians who have not completed a residency training programme are not eligible. That is the opposite of the Family Medicine Loan Repayment Incentive, which is open only to residents still in training. What are the TSLRP hours requirements?
Full-time means forty hours a week with no more than eight hours spent on practice-related administrative duties. Part-time is twenty to thirty-nine hours a week, with a choice of a two-year or four-year service obligation. The minimum service agreement is two years.Does an FQHC placement qualify for TSLRP?
The current announcement, revised November 2024, conditions the eligible practice site on federal Health Professional Shortage Area designation and no longer carries the Federally Qualified Health Center or Rural Health Clinic language that an older, still-published version of the same document contained. Confirm the specific site with the Tennessee Department of Health.Mike Certo · NMLS #260555 · Cornerstone First Mortgage NMLS #173855 · Equal Housing Lender.
